Do you really need supplements?
Probably not most of them. Here is how to tell which question you are actually asking, and who can answer it.

Nobody in the shop is going to tell you no
Stand in the supplement aisle for five minutes and you will be told, by the packaging alone, that you are tired, stressed, under-slept, ageing badly, and low on at least four things you cannot pronounce. Every box is a small argument that something is missing from you. None of them can see you.
That is the honest starting point. The question "do I need supplements" almost never has a single answer, because it is really three questions wearing one coat: am I short of something, would a pill fix it, and is this particular pill the way to do that. They have different answers and different people who can answer them.
"Food first" is not a slogan, it is a description of how absorption works
For most people eating a reasonably varied diet, the majority of everyday nutrient needs are already met. That is not wishful thinking - it's simply how a varied diet tends to work in practice. But the more interesting part is why food tends to work well, because it is not simply that food is wholesome.
A meal delivers nutrients alongside the things that help you use them. Fat in the meal is what carries vitamins A, D, E and K across the gut wall, which is why a vitamin D capsule swallowed with black coffee behaves differently from egg yolk eaten with breakfast. Vitamin C in the same plate as beans measurably improves how much of the plant iron you absorb. Fibre, water and volume also decide how full you feel, and no capsule does that.
A supplement is a patch for a specific hole. It is a poor substitute for a floor.
This is also why "I take a multivitamin so my diet does not matter" tends to disappoint. The multivitamin covers a list. The meal covers the list plus the context.
Six situations where the question is genuinely fair
None of these mean you are deficient. A blood test ordered by a clinician is the only thing that establishes that. What they mean is that the odds have shifted enough to be worth a conversation rather than a guess.
- You eat no animal foods at all. Vitamin B12 occurs naturally in almost nothing else, and fortified products are the only reliable plant-side source.
- Your days are almost entirely indoors. Skin, not the plate, is where most vitamin D comes from.
- You are pregnant, planning to be, or breastfeeding. Requirements genuinely change here, and this is squarely a clinician's territory.
- You cut out a whole food group, whether by choice, allergy, or an intolerance you have been managing for years.
- You take medication long term. Some medicines change how much of a nutrient you absorb or keep, which your pharmacist can tell you about in about two minutes.
- Your appetite or your ability to eat normally has changed, after illness, surgery, or a stretch of eating very little.
The thing most labels do not say out loud
In most markets, supplements are regulated as food rather than as medicine. That means a product does not have to be approved for effectiveness before it goes on sale the way a prescription drug does. Responsibility for the claims sits mainly with the company printing them.
This is not a scandal and it does not make supplements bad. It just means the burden of judgement lands on you in a way it does not at the pharmacy counter. It is a good argument for reading the ingredient panel rather than the front of the box, and for treating a confident claim as marketing until something independent backs it.
Why the right answer changes depending on where you live
Here is a concrete example of how location-bound this gets. The NHS advises that everyone in the UK consider a daily vitamin D supplement during autumn and winter, because at that latitude the sunlight simply is not strong enough for months at a time. That is sensible, specific public health advice. It is also close to meaningless if you live in Bangkok.
The reverse also happens: someone in a sunny country who commutes underground, works in an office until dark, and covers up outdoors can end up in a similar position to a Londoner in January. Latitude sets the ceiling. Your week decides how much of it you actually collect.
So when you read a supplement recommendation online, the first useful question is not whether it is true. It is who it was written for.
What a supplement cannot do, no matter what it costs
- It cannot tell you that you were deficient. Only a test ordered by a healthcare professional does that, before and after.
- It cannot make more of itself useful by being bigger. Past the point your body can use, extra is either excreted or, for the fat-soluble ones, stored.
- It cannot replace treatment for a diagnosed condition, and it cannot be assumed safe alongside your medication without checking.
- It cannot fix the thing that is usually actually wrong, which is sleep, or stress, or not eating enough during the day.
That last one deserves saying plainly, because it is the most common. A great many people arrive at the supplement aisle looking for energy when what has actually changed is that they are sleeping six hours and eating lunch at their desk at four in the afternoon.
A reasonable way to decide
Start by writing down what you actually want to change, in ordinary words. "I am flat every afternoon" is a better starting point than "I think I need B vitamins", because the second one has already quietly answered the question for you.
Then look at the week around it: how you sleep, what a normal day of eating looks like, how much of it happens outdoors. Then take that, not a product name, to a doctor, pharmacist or dietitian. It is a five-minute conversation and it is free at most pharmacy counters.
You may well come out of it taking nothing. That is a real answer, and it is the one this article is quietly hoping for.
